GRIT SC LLC – Sports Activities & Open Clinics

By signing below, I agree to the following:

LIABILITY WAIVER

I hereby release and hold harmless GRIT SC LLC, its directors, coaches, employees, and volunteers from any and all liability for any injury, accident, loss, or harm that may occur during activities organized by the club, whether at rented facilities, external venues, or any affiliated location.

I understand that GRIT SC has taken reasonable precautions to ensure participant safety, and I voluntarily assume all risks associated with my child’s participation.

 MEDICAL CONSENT

I authorize GRIT SC staff or representatives to make emergency medical decisions in the event of an injury or accident involving my child, should I not be immediately reachable. This includes transportation to a medical facility, basic first aid, or any other necessary actions to protect my child’s well-being.

I confirm that my child is in good physical condition to participate in sports activities, and I have informed the club of any relevant medical conditions.

MEDIA RELEASE

I give GRIT SC LLC permission to photograph, film, or record my child and to use such materials for official purposes, including but not limited to:

• Club social media accounts
  
• GRIT SC website
  
• Promotional materials (digital or print)
  
  This authorization has no expiration date and does not entitle me or my child to any form of compensation.
  GRIT SC agrees to use this material respectfully and will not publish content that could compromise the dignity or privacy of the child.

This consent may be revoked in writing at any time, except for materials already published.
